Overview of Conditions:

Lumbar discomfort is a prevalent musculoskeletal issue, often categorized into acute (less than 6 weeks), subacute (6-12 weeks), and chronic (over 12 weeks) conditions. Causes vary, including muscle strain, ligament injuries, degenerative disc disease, disc herniation, spinal stenosis, or poor posture.

Pathophysiology involves mechanical or inflammatory processes affecting the musculoskeletal system. Factors like muscle imbalances, poor posture, prolonged sitting, repetitive strain, and trauma can worsen the condition.

Assessment and Evaluation:

A thorough assessment helps identify the underlying cause and appropriate treatment.

Patient History:

  • Duration, nature, and onset (acute, chronic).
  • Factors that exacerbate or relieve discomfort (e.g., sitting, bending, lifting).
  • Previous treatments or injuries.
  • Functional limitations (e.g., difficulty standing, walking, bending).

Physical Examination:

  • Palpation for tenderness, muscle tightness, and spasms.
  • Postural assessment (e.g., forward head posture, lumbar lordosis).
  • Range of motion (ROM) testing (flexion, extension, rotation).
  • Neurological examination to check for radiculopathy (numbness, tingling, or weakness in legs).
  • Special tests (e.g., Straight Leg Raise, Slump Test) for nerve root involvement.

Functional Assessment:

  • Functional movement assessment (e.g., bending, lifting, squatting).
  • Pain assessment using a Visual Analog Scale (VAS).

Goal Setting:

  1. Pain Relief:

    • Reduce intensity and frequency using modalities like heat, electrotherapy, or manual therapy.
  2. Restore Mobility:

    • Improve range of motion in the lumbar spine, pelvis, and hips.
  3. Strengthening:

    • Strengthen the core and spinal stabilizers to support the spine and prevent recurrence.
  4. Functional Restoration:

    • Restore daily functional activities (e.g., walking, sitting, lifting) to normal.
  5. Prevention:

    • Educate on posture, ergonomics, and lifestyle changes to avoid future episodes.
