Neuroplasticity is the brain’s ability to reorganise itself — to strengthen existing connections and form new ones in response to experience and practice. It is the reason rehabilitation works at all.

What it actually means
A stroke damages an area of brain tissue, and that tissue does not grow back. What can change is how the rest of the brain organises itself around the loss. Undamaged regions can take on more of the work, and connections that are used repeatedly become more efficient.
This is not mystical and it is not automatic. It is driven by what you do, repeatedly and specifically.
What drives it
- Repetition. Change follows practice, and meaningful practice usually means many repetitions — far more than most people expect.
- Specificity. The brain gets better at what it actually practises. Practising standing improves standing; it does not automatically improve reaching.
- Difficulty. Tasks need to be hard enough to challenge but achievable enough to succeed at reasonably often.
- Meaning. People practise more, and attend better, when the task matters to them.
- Use. An affected limb that is not used tends to be used even less over time. Structured programmes work against that.
What neuroplasticity does not mean
It does not mean the brain will repair itself given enough time. It does not mean every function can be recovered. And it does not mean that any particular therapy, device or supplement “unlocks” recovery — be cautious of anything sold on that claim.
It means that the work you put in has a biological reason to help. How much it helps varies between people, and depends on the stroke itself.
What this means for you practically
The single most useful implication is that what happens between therapy sessions matters as much as the sessions. An hour of therapy a week will not, on its own, deliver the volume of practice the brain needs. A good programme gives you something specific to do daily.
Frequently asked questions
Does neuroplasticity stop after six months?
No. It is most active early, which is why early rehabilitation matters — but the brain remains capable of change well beyond that.
Can neuroplasticity be increased with medication or supplements?
There is no supplement that reliably does this. Be sceptical of products claiming otherwise, and speak to your doctor before taking anything new.
